Let’s start by looking at why the gums become more sensitive in the first place.

Hormonal Changes and the Mechanism Behind Pregnancy Gingivitis

The rise in progesterone and estrogen during pregnancy widens the blood vessels in the gums and increases blood flow there. As a result, the gums can feel more swollen than usual and tend to bleed more easily when you brush your teeth.

In my own consultations with pregnant women, I’ve heard many times, “my gums suddenly started bleeding.” In most cases this is a temporary change during pregnancy that naturally settles down once hormone levels stabilize after delivery. On X, @kakkun73411 shared that during a regular dental checkup in the third trimester, the dentist explained that “a lot of pregnant women get swollen gums around this stage, so it’s nothing to worry about.” Simply knowing this is a change specific to this stage of pregnancy can already make you feel more at ease.

That said, it’s not a good idea to dismiss swelling or bleeding as “something that happens to everyone during pregnancy” and leave it alone. If the inflammation in the gums continues, it can progress into gum disease and even affect the bone that supports your teeth. If swelling or bleeding is bothering you, talk to your dentist sooner rather than later.

 <rect x="0" y="0" width="720" height="220" fill="#fff9fb"/>
 <rect x="20" y="70" width="200" height="80" rx="10" fill="#f9cede"/>
 <text x="120" y="105" text-anchor="middle" font-size="17" fill="#5a2b3a">Rise in female hormones</text>
 <text x="120" y="130" text-anchor="middle" font-size="13" fill="#9a6070">Progesterone / estrogen</text>

 <rect x="280" y="70" width="180" height="80" rx="10" fill="#f6b8cd"/>
 <text x="370" y="105" text-anchor="middle" font-size="17" fill="#5a2b3a">Increased blood flow</text>
 <text x="370" y="130" text-anchor="middle" font-size="13" fill="#5a2b3a">to the gums</text>

 <rect x="520" y="70" width="180" height="80" rx="10" fill="#f28aab"/>
 <text x="610" y="100" text-anchor="middle" font-size="17" fill="#fff">Pregnancy gingivitis</text>
 <text x="610" y="123" text-anchor="middle" font-size="13" fill="#fff">Swelling and bleeding</text>
 <defs><marker id="arrow1" markerWidth="10" markerHeight="10" refX="8" refY="3" orient="auto"></marker></defs>
 <text x="360" y="195" text-anchor="middle" font-size="14" fill="#9a6070">This change often eases once hormone levels settle after delivery</text>

How pregnancy gingivitis develops (from hormonal change to gum inflammation)

What Happens If Gum Disease Is Left Untreated? This figure is reported to be higher than other risk factors such as smoking or advanced maternal age. One reason cited is that periodontal bacteria and inflammatory substances in the mouth can travel through the bloodstream and reach the placenta.

Numbers like these can sound alarming, but gum disease is a condition that can be treated and prevented. Keeping up with daily care and regular dental visits covers most of what you need to manage it. Try not to be overly fearful — the key is simply to bring up any symptoms that concern you sooner rather than later.

When Can You Get Dental Treatment? It’s extremely rare for a standard medical X-ray examination to reach that level. The radiation dose from a single dental X-ray is only a tiny fraction of that threshold.

It’s common practice to use a protective apron over the abdomen during the X-ray. If you’re still concerned, don’t hesitate to ask your dentist to explain on the spot.

Is It Safe to Use Anesthesia?

Many people assume that “anesthesia and X-rays are completely off the table during pregnancy.” On X, @chack329 looked back and wrote, “I thought I couldn’t get anesthesia or X-rays while pregnant, so I just put up with it.” I’ve met many pregnant patients who delayed treatment because of the same misunderstanding.

In reality, once you’re in the second trimester, treatment using local anesthesia can usually proceed as normal in most cases. The amount of local anesthetic used in dentistry is small, and the Japan Dental Association also publishes basic information on dental anesthesia. Your dentist will decide the type and amount of anesthetic based on your condition and medical history, so don’t hesitate — just let them know you’re pregnant.

Putting off treatment while enduring pain can sometimes make stress and inflammation drag on longer instead. Once you reach the second trimester, it can be worth discussing any dental concerns you’ve been holding off on all at once. Japan’s Children and Families Agency positions this program, as part of its maternal and child health measures, as a way to detect changes in oral health, cavities, and gum disease early and provide health guidance.

When and Where You Can Get One

Many municipalities recommend having this checkup during the second trimester (roughly weeks 12–27), when your condition tends to be more stable. In some municipalities, the checkup doesn’t include an X-ray.

If the checkup flags a possible cavity or gum disease, the next step is a more detailed examination and treatment at a regular dental clinic. When your gums are more sensitive, brushing too hard can actually cause more bleeding. For plaque a toothbrush alone can’t remove, using dental floss alongside it is effective.

If you use mouthwash, choose an alcohol-free formula rather than one with a strong, irritating alcohol content. If you just want your mouth to feel fresh, simply rinsing with water is sometimes enough.

When to Brush After Morning Sickness Brings Up Stomach Acid

When stomach acid from vomiting lingers in the mouth, it can temporarily soften the enamel on your teeth. Brushing right away in this state can end up damaging the enamel, so it’s worth being careful.

Right after vomiting, rinse your mouth with water first to wash away the acid. Waiting around 30 minutes before brushing helps reduce the strain on your enamel. On days when brushing itself feels too difficult, chewing sugar-free gum to encourage saliva flow is another option.

Hydration and Nutrition When Cavities Seem to Progress Quickly

Many pregnant women drink water each time they wake up for nighttime bathroom trips, which can leave the mouth feeling dry more often. Since saliva helps wash away bacteria and acid in the mouth, ongoing dryness can make it feel like cavities are progressing faster.

On X, @nx_kcb wrote, “I drink water every time I wake up from frequent nighttime urination, but my mouth is still bone dry, and my cavities during pregnancy are progressing seriously fast.” It turns out quite a few pregnant women share this same feeling. Rinsing your mouth frequently and switching to lower-sugar drinks are both simple ways to counter dryness.

Alongside that, eating with calcium, vitamin D, and vitamin C in mind also supports healthy gums. After eating something sweet or highly acidic, get into the habit of waiting around 30 minutes before brushing.

Gum disease has also been linked to broader conditions such as pregnancy-induced hypertension. In the first trimester, treatment is usually limited to emergency care, and in the third trimester, clinics tend to avoid having you lie on your back for long periods. Since your actual treatment plan depends on your condition, let your dentist know how many weeks pregnant you are and discuss it together.

Q.
