About the treatment of minors
Please be sure to obtain the consent of the guardian before visiting the hospital for minors.
In addition, if you wish to have surgery on the day of surgery, you will need the consent form of a guardian when you come to the hospital.
Please download and print the parental consent form, fill it out, seal it, and bring it to the clinic.
