FB Points Terms of Use

The FB Points Terms of Use (hereinafter referred to as “these Terms”) stipulate the conditions for the use of “FB Points” (hereinafter referred to as “Points”) provided by Fukubikai Medical Corporation (hereinafter referred to as “our Association”) between the user (hereinafter referred to as “Member”) and our company.

Article 1 (Purpose)

These terms and conditions stipulate the terms and conditions for providing the points service (hereinafter referred to as “this service”) to users of clinics and websites operated by our Association.

Article 2 (Consent)

Members may use the Service only after agreeing to these Terms. By acquiring or using points, Members are deemed to have agreed to these Terms and are registered as Members.
２．Members are deemed to have agreed to the Terms and Conditions when using the Service, and, unless otherwise specified, as defined by the Association.

Article 3 (Changes to Terms)

The Association may change these Terms. In this case, the Association will notify the Member of the effective date of the change and the contents of the change on the website operated by the Association, and if the Member continues to use the Service after the effective date of the change, the Member will be deemed to have agreed to the change to these Terms, and the contents of the change to these Terms will come into effect between the Association and the Member.

Article 4 (Ownership of Rights)

All rights related to the Service (including, but not limited to, intellectual property rights such as copyrights, trademarks, patents, etc.) belong to the Club. Members may use points within the limits of the usage rights granted by the Club under these Terms and Conditions.

Article 5 (Acquiring Points)

The Association will award points to Members when they undergo self-paid medical treatment or testing at clinics operated by the Association, or when they shop at the Generio Store operated by the Association. The number of points available to Users can be checked on the website designated by the Association.
２．The Association will determine the recipients of the points and the number or rate of points to be awarded, and Members must comply with this.
３．If a refund occurs due to cancellation or other reasons after points have been awarded, the awarded points will be returned in an amount equivalent to the refund amount.

Article 6 (Point Management)

The Club will notify the Member of the number of points acquired by the Member, the number of points used by the Member, and the remaining number of points in the manner prescribed by the Club. If the Member has any doubts about the number of points, the Member shall immediately contact the Company and explain the content of the doubts.
２．The Club shall make the final decision regarding the number of points in the preceding paragraph, and the Member shall comply with this decision.

Article 7 (Prohibition of Transfer and Combination of Points

Members may not transfer or pawn their points to other Members, or share points among other Members.
２．If a Member has multiple membership registrations, the Member may not combine the points held in each membership registration.
3. The provisions of the preceding two paragraphs shall not apply in cases approved by the Club.

Article 8 (Cancellation of Points)

After the Club has awarded points, if there is a cancellation or other reason that the Club deems appropriate to revoke the awarding of points, the Club may revoke the points awarded for the applicable transaction.
２．If the Club determines that a Member falls under any of the following items, the Club may revoke some or all of the points held by the Member without prior notice to the Member:
(1) In the event of illegal or fraudulent conduct
(2) In the event of a violation of these Terms and Conditions or other terms and rules established by the Club
(3) In any other case that the Club deems appropriate to revoke the points awarded to the Member
３．The Club shall not provide any compensation or bear any responsibility for cancelled points.

Article 9 (Use of Points)

Members may use their points in units of 100 points, with 1 point being equivalent to 1 yen, to pay for all or part of private medical treatment and examinations at clinics operated by the Association, or for purchases at the Generio Store operated by the Association, in accordance with the method specified by the Association.
２．The minimum number of points that a Member may use for one private medical treatment or examination or purchase at the Generio Store is 100 points, and the maximum is the number of points held by the Member.
３．If a private medical treatment or examination or a purchase at the Generio Store using points is canceled, the points used for the payment will be refunded in points as a general rule and will not be refunded in cash.
４．The Association may restrict or impose conditions on the use of points for private medical treatment and examinations or purchases at the Generio Store that are eligible for the use of points.

Article 10 (Validity period and expiration of points)

There is no expiration date for points. However, points may be subject to change at the discretion of the Club.

Article 11 (Changes to the Service)

The Association may change the content of the Service or the conditions for providing the Service (including, but not limited to, abolishing points, suspending point allocation, changing target sites or transactions, and changing point allocation rates or usage rates) without prior notice to Members, and may terminate or suspend the Service. Members shall agree to this in advance.
２．The Association shall not be liable for any disadvantage or damage caused to Members by the changes in the preceding paragraph.

Article 12 (Not exchangeable for cash)

Members cannot redeem points for cash under any circumstances.

Article 13 (Use by Third Parties)

Points must be used by the Member himself/herself and may not be used by any third party other than the said User.
２. Use of the points confirmation site will be deemed to have been made by the Member himself/herself at the time the User ID and associated password are entered. Even in the event of unauthorized use by a third party, as long as the User ID and associated password for this site are entered correctly, the Association will not return the points used and the Association shall not bear any responsibility in relation to this.

Article 14 (Taxes and Fees)

If taxes or other incidental expenses arise in connection with the acquisition or use of points, the Member shall bear these.

Article 15 (Termination or Suspension of Membership）

If a Member loses his/her membership status, he/she will lose all points and other rights relating to the use of the Service and will not acquire any claims against the Company as a result of the loss of his/her membership.

Article 16 (Disclaimer)

The Association will do its best to operate the Service based on the current state of technology, but does not guarantee that there will be no problems. The Association shall not be liable for any interruption, delay, or cessation of the system or loss of data due to failures in communication lines or computers, failures in point usage, damages caused by unauthorized access to data, or other damages incurred by members in relation to the Service.

Terms of Use Clarification

• 1 point will be awarded for every 100 yen (excluding tax) spent on self-paid medical treatment or self-paid examinations at clinics operated by the Association, or for purchases at the Generio Store operated by the Association.
• NIPT testing gives you 1 point for every 20 yen spent
• 1 point can be used as 1 yen
• Use in increments of 100 points
• Points will be awarded within one week of using the service and can be used from the date of award.
